Information for Patients with appointments

Dear Patients,

We have put together some new guidelines for our practice, to keep you and our staff safe. We would appreciate if you would read through these before attending your appointment.

If any of the following apply, do not attend the practice:

• you are suffering from Covid-19 symptoms

• living with someone with Covid-19 symptoms

• you have been contacted by Test, Track & Trace

If any of the following apply, patients should follow government advice.

• vulnerable (stringently safe distancing)

• extremely vulnerable (shielding) – current advice is to stay at home until end of June – book at beginning of session

If you are happy to have a face-to face appointment, we will try to accommodate you at the beginning of a session.

Treatment will be limited in this phased return

Please arrive alone; if essential, only one carer or parent may accompany you, wearing a face covering.

Please only bring essential items with you (eg no bags, shopping, umbrellas, magazines etc)

Please DO NOT arrive more than 5 minutes before your appointment time, because we have limited room for safe distancing.

Enter the building via the doors from the road and wait in the foyer at the bottom of the staircase.

If you have a mobile phone, please telephone reception on 0121 550 1080 to announce your arrival and receive instructions

If you don’t have a mobile phone please go up the stairs and use the intercom system to announce your arrival and receive instructions Please do not wait on the staircase.

DO NOT wear gloves, but wear a face covering if you have one.

Please bring a pen with you. When you are called through to the practice, the nurses will ask you to use hand gel with the gel provided.

Please keep a social distance and the team will call you through as soon as the surgery and clinician are ready

Please keep your mobile phone on so we can contact you if needed.

In the surgery, we will not be able to safe-distance from you but we will be wearing PPE when you enter.

When your treatment is finished, payment (if applicable) should be contactless if possible.

Patients will not be able to use the toilets except in emergency.

We thank you for your cooperation whilst we return to a phased new normal.

Your MDP Team.
